San Bernardino County’s environmental health department did not report shutting down any restaurants or other food facilities because of imminent health hazards between Feb. 18 and 24, 2022.
Here are selected inspections at facilities that weren’t closed but had other significant issues.
Tai Pan House, at 8031 Archibald Ave. in Rancho Cucamonga, which received a grade of 87/B on Feb. 4, requested another inspection. This time, on Feb. 23, it received an 80/B with three critical violations. An employee handled raw beef and then touched several surfaces after rinsing but not washing his hands. Tamales had been left out at room temperature for at least two hours. And sanitizer wasn’t being used to clean a cutting board. Among the six other violations, cooked lamb wasn’t being cooled down fast enough. This was the restaurant’s fourth B or C grade since 2017.
El Potrero, at 26483 Base Line Suite C in Highland, was inspected Feb. 22 and received a grade of 82/B with two critical violations. Several items of food were at unsafe temperatures in a prep cooler that wasn’t keeping cold, while carne asada and el pastor weren’t being kept hot enough and cheese had been left at room temperature. Also, a spatula wasn’t washed properly. Among the eight other violations, the manager didn’t know the correct cooking temperature for chicken, or proper hot holding temperatures.
Tasty House, at 2947 Chino Ave. Suite 3 in Chino Hills, was inspected Feb. 22 in response from a complaint from someone who said two people got sick after eating rice from the restaurant. The person in charge said after rice is cooked, it’s left on the counter for a couple of hours to cool down. They also didn’t know proper reheating or hold holding temperatures. The restaurant received a grade of 83/B with one critical violation, for tofu and cooked noodles being at unsafe temperatures. Among the 10 other violations, cooking equipment and the floors needed cleaning. This was the restaurant’s third consecutive B grade, and sixth B or C grade since 2017.
Mimi’s Cafe, at 16933 Sierra Lakes Parkway in Fontana, which received a grade of 80/B on Dec. 13, requested another inspection. This time, on Feb. 22, it received an 83/B with two critical violations. Some pasta wasn’t being kept cold enough. Also, the dishwasher wasn’t dispensing sanitizer — its hose had a leak — and there was buildup on the soda gun nozzle. The inspector returned the next day and confirmed the dishwasher was fixed.
MGM Burgers, at 11515 Cedar Ave. in Bloomington, which received a grade of 76/C on Feb. 2, underwent a mandatory rescore inspection Feb. 22. This time it received an 89/B with one critical violation, for rice being at an unsafe temperature for too long. Among the four other violations, there was mold in the ice machine (not contaminating ice) — a repeat violation from Feb. 2. This was the restaurant’s fourth B or C grade since 2019
Troy’s Burgers, at 4750 Riverside Drive in Chino, was inspected Feb. 18 in response to a foodborne illness complaint. It received a grade of 84/B with two critical violations. A person handling raw meat wasn’t using gloves properly to prevent cross-contamination, and some food was at unsafe temperatures in a refrigerator that wasn’t keeping cold. This was the restaurant’s second B grade since 2021.
Panda Bowl, at 1213 Wabash Ave. in Redlands, was inspected Feb. 17 and received a grade of 72/C with three critical violations. Cooked chicken wasn’t being kept hot. There was mold in the ice machine, with condensation dripping over it into the ice. Also, the facility’s water heater wasn’t strong enough to keep the water hot when several sinks were run simultaneously. Staff was told to minimize the number of sinks turned on at the same time until they could get a new water heater. Among the 12 other violations, the inspector found three dead cockroaches and some old rodent droppings, but said a thorough search showed no signs of an active infestation. This was the restaurant’s third B or C grade since 2019.
Hime Sushi & Roll, at 4110 Edison Ave. Unit 104 in Chino, was inspected Feb. 16 and received a grade of 73/C with two critical violations. Two employees didn’t wash hands before preparing sushi rolls and after handling raw shrimp. Also, a cutting board and knife at the sushi bar were being wiped down without sanitizer. Among the 14 other violations, some crab wasn’t being kept cold enough, no one was keeping track of how long other food had been left at room temperature and an employee had unrestrained medium-length hair. This was the restaurant’s third B or C grade since 2020.
Legends Burgers, at 1645 N. Mountain Ave. in Upland, which was closed Feb. 16 because the walk-in refrigerator and another fridge weren’t working, was permitted to reopen Feb. 17.
This list is published online on Fridays. Any updates as restaurants are reopened will be included in next week’s list.
All food facilities in the county are routinely inspected to ensure they meet health codes. A facility loses four points for each critical violation and one to three points for minor violations. An A grade (90 to 100 points) is considered “generally superior,” a B grade (80 to 89) is “generally acceptable” and a C grade (70 to 79) is “generally unacceptable” and requires a follow-up inspection. A facility will be temporarily closed if it scores below 70 or has a critical violation that can’t be corrected immediately.
